
Photography and Video Documentation Policy
The Photography and Video Documentation Policy at California Hair and Skin Institute applies to all physicians, nurses, technicians, medical assistants, administrative personnel, contractors, and third-party vendors who participate in patient photography or video documentation.
Authorized Uses
Clinical photography and video may be used for baseline assessment, treatment planning, progress monitoring, surgical documentation, medical record documentation, quality assurance, education, professional presentations, scientific publications, website content, social media, YouTube, TikTok, and marketing when proper authorization has been obtained.
Consent Requirements
Clinical photography consent is required for medical record documentation. Separate written authorization is required for marketing, advertising, website publication, social media use, testimonials, and promotional materials. Patients may decline marketing authorization without affecting treatment.
Patient Identification
Patient name, date of birth, and medical record number are verified before obtaining photographs. Images are linked to the correct patient record.
Photography Equipment Standards
Approved equipment includes professional DSLR cameras, mirrorless cameras, approved clinic mobile devices, and professional lighting systems maintained according to manufacturer recommendations.
Photography Environment
Neutral backgrounds, consistent lighting, standardized positioning, and controlled image capture conditions are used to ensure reproducible results.
Standardized Hair Restoration Views
Required views include frontal, right oblique, left oblique, right lateral, left lateral, vertex, and donor area photographs.
Standardization Requirements
Consistency is maintained in camera distance, camera angle, lighting, background, hair length when possible, and patient positioning. Digital enhancement or misleading image manipulation is prohibited.
Pre-Procedure Photography
Baseline photographs including frontal, donor, crown, and treatment planning views are obtained prior to treatment.
Intraoperative Photography
May include hairline markings, donor harvesting, recipient site creation, graft placement, and final operative results when appropriate consent is documented.
Post-Procedure Photography
Recommended intervals include immediate post-procedure, month 4, month 8, and month 12 follow-up documentation.
Video Documentation
Video recordings may be used for surgical documentation, educational content, testimonials, and marketing activities when authorized by the patient.
HIPAA Compliance
Photographs and videos containing identifiable patient information are treated as protected health information and stored using secure, password-protected, access-controlled systems.
Social Media Policy
Staff shall not post patient photographs or videos without authorization, use personal accounts for clinical images, or transmit images through unsecured channels. Only authorized personnel may publish content.
Data Retention
Clinical photographs and videos are retained in accordance with California medical record retention requirements and clinic policy.
Quality Assurance
Quarterly review evaluates consent compliance, image quality, documentation standards, HIPAA compliance, and storage security practices.
